If you are going into hospital for treatment or ready to be discharged and wish to ease the transition from hospital to home, Saga Homecare can help to provide flexible and bespoke support.
The service we provide will be tailored to your specific requirements and includes all personal and domestic tasks as required.
We are able to visit you at home prior to your treatment or at hospital before you are discharged and will liaise with other professionals involved in your care as appropriate to ensure we create a personalised care plan that meets all your needs.
Our carers will also be able to help with additional duties such as accompanying you to GP / Hospital appointments as required or simply providing companionship to help you get back on your feet.
